Attach the following along with the print out of the application form

• Original payment slip
• Certified copy of birth certificate & its English translation (if the original is not in English)
• Copy of the National Identiti Card (NIC)
• Certified copy of A\L result sheet (issued by the Dept. of Examinations) sheet or any other certificates.
• Certified copy of O\L result sheet (issued by the Dept. of Examinations) sheet or any other certificates (if mentioned as required in the application).
• Any other attachments mentioned in the downloaded application.
